Job Description
The Wound Care Nurse Practitioner (NP), supervised by the Director of Nursing, provides specialized wound care services to patients in home settings, nursing homes, and assisted living facilities.
They perform comprehensive assessments, develop individualized care plans, and manage treatment for complex wounds, pressure injuries, and surgical wounds. Responsibilities include patient and caregiver education, supervising nursing staff, and collaborating with healthcare teams to promote healing outcomes. The role requires a Master’s or Doctoral degree in Nursing, current licensure, Wound Care Certification within 6 months, and at least 2 years of relevant experience. Physical demands involve mobility, lifting, and exposure to various environments, with travel within the region necessary. Strong assessment, communication, and interpersonal skills are essential, along with adaptability and professionalism.
