Job Description

We are seeking a *Certified Wound Care Nurse Practitioner* for a full-time, mobile role in Bloomington, Illinois, offering $150,000/year plus bonus.

  • Provide advanced wound care for assisted living and long-term care facilities throughout the area.
  • Assess and manage chronic wounds such as DFUs, VLUs, and pressure injuries.
  • Perform sharp debridement within scope of practice.
  • Apply advanced dressings and skin substitutes.
  • Accurately document care in EMR following Medicare/LCD guidelines.
  • Collaborate with physicians and facility staff.

    • Requirements include an active Illinois NP license and wound care certification (WCC, CWCN-AP, CWS, or equivalent). Preferred candidates have 2+ years of wound care experience and LTC/SNF background.
