We are seeking a compassionate HealthCare Coordinator with an active RN or BSN license to lead clinical operations in a Memory Care Assisted Living community.
Responsibilities include conducting resident assessments, developing personalized care plans, overseeing clinical care, ensuring regulatory compliance, and maintaining a safe, respectful environment. Lead and mentor caregiving staff, manage medication processes, and collaborate with healthcare providers to support residents’ health and well-being. Promote resident-centered care with strong communication and family engagement, fostering trust and transparency. Ensure quality standards, participate in quality improvement, and support team development.
