Job Description
The Home Health Speech-Language Pathologist (SLP) provides evidence-based speech, language, cognitive, and swallowing therapy to patients in their homes.
Conducts comprehensive assessments and develops personalized care plans aligned with physician orders and regulatory standards. Implements therapeutic interventions, educates patients and families, and adjusts treatment based on progress. Ensures compliance with CMS Conditions of Participation, documentation, infection control, and billing requirements. Collaborates with interdisciplinary teams to coordinate care and optimize outcomes. Provides patient and caregiver education on communication and swallowing strategies. Participates in quality improvement initiatives and maintains accurate documentation. Requirements include valid Ohio licensure.
