Job Description

Seeking a Speech Language Pathologist for our Home Care team to provide personalized evaluations and treatments in patients’ homes, enhancing communication, cognition, and swallowing capabilities.

Responsibilities include conducting assessments, developing evidence-based treatment plans, documenting progress, educating patients and caregivers, and collaborating with a multidisciplinary team.

Qualifications: Master’s in Speech-Language Pathology, CCC or CFY, NY State licensure, and CPR certification; experience with dysphagia and neurogenic disorders preferred.
