Job Description
We are seeking a Home Health Speech Language Pathologist to assess and treat speech, language, social communication, cognitive, and swallowing disorders through home visits.
- Perform initial assessments, develop and update individualized care plans, and conduct re-evaluations every 30 days.
- Coordinate care with healthcare team members and document patient progress accurately.
- Educate patients and caregivers, providing support and educational materials to promote independence.
- Complete assessments and data collection as required.
Qualifications include a Master’s degree in Speech Language Pathology, active license (CCC), at least one year of experience, and familiarity with home health documentation.
