Nurse Practitioner – Spanish Required

Job Description

We are seeking a Nurse Practitioner to provide high-quality, value-based primary care for adult Medicare patients in an outpatient setting.

Independently conduct office visits for routine and acute issues, including wellness exams and health assessments.

Collaborate with a multidisciplinary team to ensure comprehensive patient care and coordinate with specialists and facilities.

Lead population health initiatives, promote patient education, and assist with outreach and home visits as needed.

Build trusting relationships with patients to support long-term health and well-being.

Qualifications include: MS or DNP, national certification (FNP, AGNP, ANP, GNP), active NP license, DEA license, and US work authorization. Preferred experience in primary care or geriatrics, with knowledge of coding and risk assessments.
