Field Nurse Practitioner

Job Description

Nurse Practitioners in this role provide in-home care for high-risk, high-cost patients on AHCCCS and Medicaid plans, focusing on improving medical outcomes and supporting patient goals.

They conduct patient assessments, review tests, create treatment plans, prescribe and reconcile medications, and educate patients and families.

Collaborating with primary care providers, specialists, and care team members, they ensure comprehensive care coordination and transitional support.

Each day involves managing around five patients, overseeing a population of 100 members monthly, and working closely with multidisciplinary teams to address barriers and optimize care.

Applicants need an active Nurse Practitioner license, experience in primary or acute care, and strong communication skills. Bilingual in Spanish is preferred.
