Job Description

The Primary Care Physician provides comprehensive, patient-centered care as part of a multidisciplinary team in an urban community health setting.

Responsibilities include developing and supporting team-based preventive and chronic care models, managing a patient panel, reviewing labs and imaging, coordinating referrals and transitions, and ensuring timely documentation.

Qualifications include MD/DO with residency in Internal or Family Medicine, board eligibility, active MA licensure, bilingual Spanish preferred, and strong communication and organizational skills.
