Job Description

Seeking a compassionate Master of Social Work (MSW) for a full-time, salaried role in Springfield, MA, supporting elderly adults within a PACE program.

  • Conduct psychosocial assessments and develop personalized care plans.
  • Provide counseling, advocacy, and support to participants and families.
  • Collaborate with an interdisciplinary team to coordinate comprehensive care.
  • Assist with crisis intervention, discharge planning, and resource referrals.
  • Ensure compliance with relevant regulations and documentation standards.
