Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or Physician Assistant responsible for independently managing patient care in PALTC settings to enhance quality, outcomes, and patient/family experience, while reducing hospitalizations and costs.
Responsibilities include conducting assessments, diagnosing and treating illnesses, developing care plans, and providing preventive services. The AP collaborates with facility leadership and interdisciplinary teams, ensuring regulatory compliance and accurate documentation. They participate in infection control, health education, and quality improvement initiatives.
Key qualifications involve a relevant degree, at least 2 years of experience (long-term care preferred), and licensure as an APRN or PA. Skills include strong clinical decision-making, communication, technology proficiency, and the ability to work independently in a fast-paced environment.
