Nurse Practitioner – Spanish Required

Job Description

We are seeking a Nurse Practitioner to provide equitable, value-based primary care for adults on Medicare in outpatient settings.

Independently manage patient visits, perform wellness exams, and coordinate care with specialists and support teams.

Build lasting patient relationships through holistic care, education, and outreach efforts, including home visits when needed.

Collaborate with a multidisciplinary team to focus on population health and preventive services.

Report to the Center Medical Director and work closely with operational leadership.

Qualifications include: Master’s or Doctorate in Nursing, active NP license, DEA license, and certification in relevant specialties. Preferred experience in primary care and familiarity with population health metrics.
