Job Description

Seeking a dedicated Neurology PA-C or Nurse Practitioner for a field-based role in SNFs and ALFs.

  • Perform comprehensive neurological assessments, diagnose and manage neurological conditions, and develop individualized care plans.
  • Order and interpret diagnostic studies, prescribe medications, and support transitional care from hospitals.
  • Collaborate with facility staff, communicate with families, and participate in interdisciplinary meetings.
  • Ensure accurate documentation, adhere to regulations, and contribute to protocol development.

    • Requirements include licensure, certification, DEA registration, with 1-2 years of relevant experience preferred. Strong clinical reasoning, communication skills, and ability to work independently are essential.
