This role involves
supervising social service programs, assessing residents’ needs, and developing care plans that are reviewed quarterly. Making arrangements for adaptive equipment, personal items, and maintaining family contact. Referring residents to community resources, assisting with financial/legal matters, and coordinating discharge or admission processes. Ensuring compliance with regulations and documentation requirements.Salary: $45,000 – $64,000/year (negotiable)
