Job Description

We are a rapidly growing provider of innovative medical care for long-term and post-acute patients across the U.S., utilizing advanced technology to prevent hospitalizations.

We seek a full-time on-site Nurse Practitioner or Physician Assistant to deliver care in Skilled Nursing Facilities in Corsicana, Texas. No weekends, nights, call, or holidays required.

Responsibilities include conducting health assessments, developing care plans, managing chronic and acute conditions, ordering diagnostics, prescribing treatments, and collaborating with interdisciplinary teams. Providing resident and family education, participating in quality initiatives, and maintaining thorough documentation are essential.

Requirements: Current Texas NP licensure, board certification, geriatric experience preferred, strong assessment and communication skills, proficiency with EMR systems, and ability to travel occasionally.
