Full-time Nurse Practitioner position focused on hospice, palliative, and house call care in Treasure Valley, ID.

The NP collaborates with a multidisciplinary team to provide in-home and facility visits, focusing on advanced care planning and symptom management to support aging in place. The role ensures continuity of care through face-to-face visits and symptom management, working closely with the hospice team.
