Seeking a qualified Nurse Practitioner to provide patient care in Beaufort, SC, primarily in patients’ homes.

As a key team member, you will evaluate high-risk patients, develop care plans, and coordinate with healthcare professionals to improve outcomes.

Responsibilities include conducting visits within designated areas, participating in interdisciplinary team meetings, documenting activities, and educating patients and families.
