About the role:

• Conduct assessments and develop personalized wound care plans
• Provide hands-on bedside wound treatment in skilled nursing and assisted living facilities
• Utilize advanced tools and evidence-based protocols to promote healing
• Collaborate with facility teams and families for holistic care decisions
• Maintain accurate EMR documentation and support quality initiatives

Qualifications:

• Active NJ Nurse Practitioner license (board-eligible not accepted) or Physician Assistant
• CDS and DEA licenses (must obtain within training period)
• Minimum 2 years wound care experience and prior SNF or long-term care exposure preferred
• Willingness to travel within Ocean County and comfortable with EMR documentation
