Job Description

The Wound Care Nurse Practitioner reports to the Director of Nursing and supervises nurses and medical assistants.

  • Provides specialized wound care in home, nursing home, and assisted living settings for chronic and complex wounds.
  • Conducts assessments, develops care plans, educates patients/caregivers, and collaborates with healthcare teams to promote healing.
  • Requires a Master’s or Doctoral degree in Nursing, valid NP licensure, wound care certification within 6 months, and minimum 2 years of relevant experience.
  • Field-based role involving travel, physical activity, and exposure to biohazards, with necessary physical and mental stamina.
  • Performs procedures, maintains documentation, ensures compliance with safety and HIPAA policies, and coordinates care with providers.
  • Strong assessment, communication, and adaptability skills are essential.
