Join a multidisciplinary practice providing exceptional post-acute care in cardiology, nephrology, or pulmonary medicine as a Nurse Practitioner – Post-Acute Care (Nephrology Specialty).

This part-time role offers up to $100,000/year with flexible hours, no weekends, and minimal on-call requirements. You will perform patient rounding and management at SNFs, ALFs, and LTACHs, delivering comprehensive care, developing care plans, and collaborating with teams and families.
