Job Description
The Licensed Mental Health Professional will be a key member of a new Safe Options Support (SOS) team dedicated to providing comprehensive care to homeless individuals living on the streets or in subways.
The multi-disciplinary team includes clinicians, care managers, a team leader, and a peer specialist, using an evidence-based Critical Time Intervention model.
The role involves community outreach, coordinating needs pre- and post-housing, enhancing daily living skills, offering supportive counseling, and advocating against discrimination and healthcare disparities.
Focus on member choice, harm reduction, non-coercion, and person-centered care is essential.
Post-housing, the team supports participants for several months to ensure stability and independence.
The position requires fieldwork, occasional on-call hours, flexible schedules, and travel across three counties, with relocation to Norwich, NY, before starting.
