About the Role
- Conduct assessments and develop personalized treatment plans
- Deliver bedside wound care in skilled nursing and assisted living facilities
- Lead wound assessment, treatment, and advanced procedures
- Utilize technology and evidence-based protocols to promote healing
- Collaborate with facility teams and families to ensure holistic care
- Maintain accurate charting in a customized EMR
- Support quality initiatives and the company’s mission to enhance post-acute care
Qualifications
- Active Pennsylvania Nurse Practitioner license (board-eligible not accepted) or Physician Assistant
- Prescriptive authority and DEA license (must obtain by end of training)
- Minimum 2 years wound care experience, preferably in SNFs or long-term care
- Willingness to travel within designated counties
- Comfort with EMR and mobile documentation
- Strong interpersonal and collaborative skills
