Nurse Practitioner – Spanish Required

Job Description

We are seeking a Nurse Practitioner to deliver equitable, value-based primary care to Medicare adults in outpatient settings.

Provide comprehensive primary care, conduct routine and acute office visits, and lead wellness assessments with thorough documentation.

Coordinate care with specialists and testing facilities, and participate in population health initiatives.

Educate patients, assist with triage and outreach, and conduct home visits as needed.

Collaborate with a multidisciplinary team including physicians, nurses, and support staff.

Qualifications: Master’s or DNP degree, national certification (e.g., Family or Adult-Gerontology Nurse Practitioner), active state license, DEA license, and US work authorization.

Preferred: Experience in primary care, familiarity with HCC/ICD-10 coding, teamwork passion, bilingual skills.
