Job Description

This role provides in-home speech therapy services to patients in the San Jose area, focusing on speech, language, cognitive-communication, and swallowing disorders.

  • Conduct initial evaluations and develop personalized care plans.
  • Deliver skilled therapy, reassess progress, and document outcomes.
  • Collaborate with an interdisciplinary team and educate patients and caregivers.
  • Complete necessary documentation, including assessments like OASIS.

    • **Qualifications:** A licensed Speech-Language Pathologist with a master’s degree and at least one year of experience; prior home health experience preferred.
