Job Description

We are seeking a compassionate Nurse Practitioner to join our PACE program, providing primary and chronic care to older adults in clinic, home, and facility settings.

  • Deliver comprehensive assessments, develop care plans, and manage medications.
  • Collaborate with a multidisciplinary team and participate in IDT meetings.
  • Educate participants and families on health, prevention, and wellness.
  • Accurately document care and support quality improvement efforts.

    • Requirements include a Master’s or Doctorate in Nursing, active NP license, and at least 2 years of geriatrics experience. Certification in Adult, Family, or Gerontology NP is preferred, along with a passion for serving seniors within a value-based care model. The role is part-time with a $75,000 annual salary and flexible scheduling. Candidates must be able to treat participants both in the clinic and at home across multiple counties in Michigan.
