Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or PA working independently to provide comprehensive medical care in PALTC facilities.

Responsibilities include conducting assessments, diagnosing and treating illnesses, managing chronic conditions, providing preventive services, and collaborating with facility staff and external providers.
They must ensure compliance, maintain accurate records, participate in quality initiatives, and promote safety and health education.
The role requires strong independent clinical decision-making, excellent communication, proficiency with EHR and telemedicine, and the ability to work in person across facilities.
Qualifications include a relevant degree, at least 2 years of experience (long-term care preferred), and active APRN or PA certification/license.
