Job Description

We are seeking a full-time Nurse Practitioner or Physician Assistant to join our Wound Care/Home Health team in Centralia, WA.

  • Work involves evaluating and managing acute, chronic, surgical, and dermatologic wounds in skilled nursing facilities, including dressing changes, debridement, and G-Tube management (training provided).
  • Opportunities for Home Health visits as needed.
  • Must have strong communication skills, ability to work independently, and experience with skilled nursing environments.
  • Preferred candidates have at least 2 years of wound care, skilled nursing, and debridement experience, and wound care certification.
  • Job offers a flexible schedule, benefits (health, dental, PTO), and a salary range of $125,000-$180,000 annually.
  • Schedule includes day shifts, Monday to Friday, with on-call and weekend coverage as needed.
