This Advanced Practice Provider role involves managing and coordinating patient care within assigned populations, supported by comprehensive training, shadowing, and oversight from a Medical Director.
Responsibilities include delivering quality care, communicating with staff and physicians, conducting patient rounds, documenting in electronic medical records, and managing billing and documentation.
Ideal candidates possess strong communication, empathy, organizational skills, and a collaborative attitude. Prior experience in geriatrics, internal medicine, or hospital settings is preferred. Required licenses include Nurse Practitioner and DEA certifications.
