Job Description
This float Nurse Practitioner or Physician Assistant role offers an exciting opportunity to impact multiple communities across Lebanon, Berks, and Cumberland Counties by providing mobile wound care at partner facilities.
Compensation & Benefits:
- Annual salary of $150,000–$155,000 with bonus potential
- $15,000 sign-on bonus

Key Responsibilities:
- Assess and develop personalized wound care plans
- Provide bedside wound care in skilled nursing and assisted living facilities
- Lead wound assessment, advanced procedures, and treatment decisions
- Utilize advanced technology and evidence-based protocols
- Collaborate with facility teams and families, maintain accurate documentation
- Participate in quality initiatives to enhance post-acute care
Qualifications:
- Active Pennsylvania Nurse Practitioner license or Physician Assistant license with prescriptive authority and DEA license (must obtain DEA by end of training)
- Minimum 2 years wound care experience, preferably in SNFs or long-term care
- Willingness to travel within designated counties
- Proficient with EMR and mobile documentation
- Strong interpersonal and collaborative skills
