Job Description

Seeking a Nurse Practitioner to deliver equitable, value-based primary care to Medicare adults in outpatient settings.

  • Provide independent clinical care, including routine and acute visits, wellness assessments, and care coordination.
  • Build patient relationships to foster trust and understanding of care plans.
  • Collaborate with a multidisciplinary team and partner with specialists for comprehensive care.
  • Lead population health initiatives and assist with outreach and education.
  • Conduct home visits and participate in promotional activities as needed.

    • **Qualifications:** Master’s or DNP degree, national certification (Family, Adult-Geron, or Gerontological NP), active NP license, DEA license, US work authorization. Experience in primary care, understanding of HCC, ICD-10, and HRAs preferred. Bilingual skills are a plus.
