Job Description
We are seeking a compassionate Nurse Practitioner to join a PACE program serving seniors across multiple Michigan counties.
In this role, you will provide primary, acute, and chronic care in clinic, home, and facility settings, collaborating with an interdisciplinary team under the Medical Director’s guidance.
Responsibilities include conducting assessments, developing care plans, managing medications, interpreting diagnostics, and coordinating care transitions. You will educate participants and families on health management, document care in the EHR, and support quality improvement efforts.
Qualifications include a Master’s or Doctorate in Nursing, an active NP license, and at least 2 years of geriatrics experience. Preferred certifications include Adult, Family, or Gerontology NP, with experience in hospice or long-term care being a plus. The role offers a part-time position with a $75,000 annual salary and flexible scheduling.
