Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or PA who independently manages patients in long-term care settings, collaborating with facility staff to improve care quality, patient outcomes, and experience while reducing unnecessary hospitalizations and costs.
Responsibilities include conducting assessments, diagnosing and treating illnesses, developing care plans, providing preventive services, and managing chronic conditions. The AP works autonomously within scope, making clinical decisions and serving as the primary provider, while building collaborative relationships with facility leadership and interdisciplinary teams.
Key duties involve ensuring compliance with regulations, maintaining accurate documentation, participating in quality initiatives, and promoting health and safety protocols. Strong communication, decision-making, and technology skills are essential, along with physical ability to respond in emergency scenarios. Qualifications include relevant degrees, licensure, and experience, preferably in long-term care.
This full-time role is based in-person at statewide PALTC facilities with benefits including health insurance, retirement plans, and paid time off.
