Job Description

Seeking a PER DIEM Speech Therapist for a Skilled Nursing Facility in Rochester, NY, located at Wesley Gardens, 3 Upton Park.

Provide quality Speech-Language Pathology services, including patient evaluation, screening, referrals, data collection, and care planning.

Complete timely documentation such as progress notes, reports, and discharge summaries.

Apply knowledge of short-term and long-term SLP care, instruct patients, family members, and staff on goals and techniques.

Ensure compliance with protocols and regulations.

Qualifications: Valid NYS License and CCC, experience with cognitive and dysphagia rehab, preferably familiarity with Rehab Optima.

Compensation: $53.00 – $58.00/hr.

Requirements: Must be able to commute to Rochester, NY 14607, and relocate if necessary before starting.
