Seeking a Senior Social Worker for a full-time position at a new Behavioral Health and Learning center in North Seattle, offering a $5,000 sign-on bonus.
Partner with patients, families, and healthcare teams to address social/emotional needs impacting care and recovery. Provide psychosocial assessments, treatment planning, discharge planning, crisis intervention, and resource referrals. Coordinate care across settings, ensuring safe transitions and effective resource utilization. Offer specialty consultation and psychosocial support, developing and implementing discharge plans in collaboration with multidisciplinary teams. Complete clinical assessments and refer patients to community resources, complying with legal reporting requirements. Qualifications: Master’s in Social Work, one year of healthcare experience, and Washington state social work credentials within 90 days.
