Job Description

We are seeking a compassionate Nurse Practitioner to join our PACE program, providing primary and chronic care to older adults across clinic, home, and facility settings.

  • Deliver comprehensive assessments, develop personalized care plans, manage medications, and coordinate care transitions.
  • Collaborate with an interdisciplinary team and participate in team meetings to ensure person-centered care.
  • Educate participants and families on health conditions, prevention, and wellness strategies.
  • Accurately document care in the EHR and support quality improvement efforts.

    • Qualified candidates will have a Master’s or Doctorate in Nursing, an active NP license, and experience in geriatrics or related fields, with a passion for serving seniors. The role is part-time, with a $75,000 annual salary and a flexible schedule. Must be willing to treat participants both in the clinic and in their homes across multiple counties in Michigan. Prior experience in geriatrics (2+ years) and hospice (preferred) is required.
