Nurse Practitioner – Spanish Required

Job Description

We are seeking a Nurse Practitioner to provide equitable, value-based primary care for Medicare adults in outpatient settings.

Independently conduct routine and acute office visits, wellness exams, and health assessments.

Coordinate care with specialists, testing facilities, and care teams.

Lead population health initiatives, educate patients, and participate in outreach and home visits.

Work collaboratively with a multidisciplinary team including physicians, nurses, and support staff.

Report to the Center Medical Director and support operational goals.

Qualifications: Master’s or DNP degree, active NP license, national certification, DEA license, and US work authorization. Preferred experience in primary care, geriatrics, or family medicine, with knowledge of HCC and ICD-10 coding.
