Nurse Practitioner – Geriatric Primary Care (SNF, LTC, ALF)

Job Description

We are seeking an experienced Nurse Practitioner to provide comprehensive primary care in Skilled Nursing Facilities, LTC, and ALFs.

Perform patient assessments, diagnose, manage, and treat acute and chronic conditions.

Order diagnostics, develop treatment plans, and prescribe medications following guidelines.

Educate patients, families, and staff on health and prevention.

Collaborate with care teams to ensure seamless, patient-centered care.

Maintain accurate documentation and support quality improvement initiatives.

Qualifications: Minnesota NP license, board certification, 1+ year experience preferred, strong geriatric and primary care background, excellent communication and teamwork skills.
