Job Description

Seeking a PER DIEM Speech Therapist for a Skilled Nursing Facility in Rochester, NY, located at Wesley Gardens, 3 Upton Park.

Provide quality SLP services, including patient evaluation, screening, referrals, data collection, and care plan follow-through.

Complete timely documentation such as progress notes, screen reports, daily notes, and discharge summaries.

Possess knowledge of short-term and long-term SLP care, and instruct patients, families, and staff on goals and therapeutic techniques.

Ensure compliance with protocols and regulatory standards.

Experience and qualifications include a valid NYS license, clinical competency certification, and preferred experience with cognitive and dysphagia rehab. Knowledge of Rehab Optima is a plus.

Compensation ranges from $53.00 to $58.00 per hour.

Candidates must be able to commute to Rochester, NY 14607, and be willing to relocate before starting.
