Seeking a Senior Social Worker for a full-time position in a new Behavioral Health and Learning center with a $5,000 sign-on bonus.
Partner with patients, families, and healthcare teams to address social/emotional needs impacting care, recovery, and transitions.
Provide assessments, treatment planning, therapeutic interventions, discharge planning, crisis support, and resource referrals.
Coordinate care across the continuum, ensuring effective utilization of resources and smooth transitions.
Offer specialty consultation and psychosocial support, develop discharge plans, and facilitate referrals to community resources.
Qualifications include a Master of Social Work, one year of healthcare experience, and state licensure within 90 days.
The role is based in a state-of-the-art facility opening in 2024, dedicated to transforming mental health care and training.
