Job Description

Seeking a Full-Time Nurse Practitioner or Physician Assistant to perform primary care visits in Assisted Living Facilities in Hyannis, MA, and nearby areas.

• Provide primary care, medication reconciliation, and annual wellness visits five weekdays.
• Examine patients, review records, order diagnostic tests, diagnose conditions, and propose treatments.
• Prescribe medications, maintain accurate documentation, and educate patients and families about health and prevention.
• Oversee overall patient health, promote wellness, and stay current with professional development and research.
