Job Description
We are seeking a compassionate Psychiatric-Mental Health Nurse Practitioner (PMHNP) to provide mental health care in skilled nursing and long-term care facilities.
Perform psychiatric evaluations and ongoing assessments. Develop and monitor individualized treatment plans with interdisciplinary teams. Diagnose and manage conditions like depression, anxiety, and dementia-related behaviors. Prescribe and adjust psychotropic medications, monitoring responses and side effects. Coordinate referrals for therapy and supportive services. Collaborate with staff to ensure quality care and provide staff education. Maintain accurate documentation and ensure regulatory compliance. Must have a Master’s or Doctoral degree, PMHNP certification, and licensure. Experience in geriatric psychiatry or long-term care is preferred.
