Clinical Manager – Registered Nurse

Job Description

We are seeking a qualified Registered Nurse (RN) to provide comprehensive patient care in a home health setting.

Conduct patient assessments, monitor vital signs, and evaluate health histories.

Administer medications and treatments following safety protocols.

Offer specialized care for internal medicine, dementia, and dialysis patients.

Perform nursing procedures such as catheterization and airway management.

Collaborate with the healthcare team to develop and document individualized care plans, including MDS documentation.

Educate patients and families on health coaching strategies.

Assist in the operating room when needed, maintaining a sterile environment.

Maintain accurate patient records compliant with healthcare regulations.

Qualifications include: a current RN license, home health experience, and preferred experience in dementia care or dialysis. Strong communication skills and ability to work within a team are essential.

This full-time or part-time role offers flexible scheduling with pay ranging from $40-$50 per hour. Preference for candidates in Fort Wayne, IN, with at least one year of dementia and home health experience.
