Job Description

Community Health is a primary care network offering comprehensive programs, including wellness, dental, behavioral health, and pediatric specialties, serving insured and uninsured populations across Rutland and southern Addison counties.


About the Role: A Licensed Mental Health Clinician specializing in pediatrics will provide mental health treatment and support to children and their families in the primary care setting, addressing social, emotional, and developmental needs.


Key Functions:

  • Offer emotional support and counseling to pediatric patients and families.
  • Conduct assessments of social, environmental, and psychosocial needs.
  • Develop individualized care plans in collaboration with healthcare teams.
  • Provide individual, family, and group therapy using evidence-based techniques.
  • Educate families on coping strategies, disease management, and available resources.
  • Coordinate with multidisciplinary teams to ensure holistic care.
  • Maintain accurate documentation and stay current with best practices.

Qualifications: Master’s degree in social work, counseling, or related field; licensed clinician (LMHC, LICSW, etc.); pediatric or primary care experience preferred; strong communication, empathy, and team collaboration skills.
