Job Description

Seeking a Full-Time Nurse Practitioner or Physician Assistant to conduct primary care visits in Assisted Living Facilities in Hyannis, MA, and nearby areas.

Responsibilities include:

  • Performing primary care visits, medication reconciliations, and annual wellness exams 5 days a week
  • Examining patients and reviewing medical records
  • Ordering and analyzing diagnostic tests
  • Diagnosing and treating health conditions and illnesses
  • Prescribing medications and maintaining accurate records
  • Educating patients and families on health issues and prevention
  • Overseeing overall patient health and promoting wellness
  • Engaging in research and professional development
