Job Description

The Certified Family Nurse Practitioner provides compassionate, patient-centered primary care services in collaboration with a healthcare team, under physician supervision.

Responsibilities include conducting wellness exams, managing acute and chronic conditions, and promoting prevention and health education, especially for underserved populations facing social barriers.

Key duties involve comprehensive patient assessments, care coordination, documentation in EHR, and participation in quality improvement initiatives.

Requires certification as a Family Nurse Practitioner, with 1 year of experience in HIV care, primary care, or community health preferred.

Offers full-time employment with a salary range of $90,000 – $100,000 and benefits such as health insurance, 401(k), paid time off, and more.
