Certified Family Nurse Practitioner (1 yr exp preferred)

Job Description

We are seeking a Certified Family Nurse Practitioner with 1 year of experience preferred to provide compassionate, patient-centered primary care services.

Assess, diagnose, and manage patients across the lifespan, focusing on wellness exams, acute illnesses, and disease management.

Collaborate with a multidisciplinary team to deliver holistic care and promote health in underserved communities.

Maintain accurate documentation in the EHR and participate in quality improvement initiatives.

Support health promotion, prevention, and population health efforts aligned with the agency’s mission.

Position details: Full-time, salary $90,000–$100,000, with benefits including health coverage, 401(k), paid time off, and more.

Requirements: Certified Family Nurse Practitioner (required) and experience in primary care, HIV care, or community health preferred.
