As a Nurse Practitioner/Physician Assistant, you will assess, manage, and coordinate healthcare for chronically ill elderly participants, collaborating with physicians and interdisciplinary teams to ensure high-quality, safe care.
Responsibilities include performing health histories, physical exams, supporting urgent care, documentation, and participating in quality improvement.
Ideal candidates are detail-oriented, analytical, organized, and confident in clinical judgment. Requirements include licensure, experience with the elderly, a valid driver’s license, and successful background checks.
