Job Description
The Home Health Speech-Language Pathologist (SLP) provides evidence-based speech, language, cognitive, and swallowing therapy in patients’ homes.
Conducts assessments, develops individualized care plans, and documents progress to ensure quality care. Implements therapeutic interventions, educates patients and families, and adjusts treatment based on progress. Ensures compliance with CMS Conditions of Participation and all relevant regulations. Collaborates with interdisciplinary teams to optimize patient outcomes. Provides patient and caregiver education on communication and swallowing strategies. Maintains accurate documentation and participates in quality improvement efforts. Requirements: Valid Ohio licensure as an SLP.
Work Conditions: Travel to patient homes, flexible hours, possible evening/weekend visits.
