Seeking a compassionate Nurse Practitioner or Physician Assistant to join our Palliative Care team.
Provides independent, patient-centered care focused on symptom management and quality of life. Conducts assessments, develops care plans, completes regulatory visits, and facilitates hospice transitions. Requires active California licensure; NP: Master’s in nursing; PA: Bachelor’s or higher, accredited PA program, NCCPA certification. Minimum two years’ experience as an advanced practice provider. Strong clinical assessment, communication, collaboration, and organizational skills. Proficiency with electronic medical records; valid driver’s license. Preferred: prior palliative care experience and board certifications. Schedule & Benefits: Expected 20–29 hrs/week, pay $59.52–$72.75/hr, with benefits including 401(k), health/dental/vision insurance, paid time off, and more.
