Job Description

Join a dedicated healthcare team as a Nurse Practitioner providing post-acute care in community facilities. This M-F 9am-5pm role involves managing patient care, collaborating with staff, conducting rounds, and documenting visits in electronic records. The position offers comprehensive training, shadowing, and oversight by a Medical Director to ensure support and preparedness.


  • Coordinate and deliver quality patient care while communicating effectively with staff and physicians.
  • Conduct patient rounds, write orders, and manage scheduling of tests and procedures.
  • Maintain accurate documentation for professional services, billing, and claims.
  • Provide services per organizational policies and regulations.

    • Requirements include strong communication, empathy, organizational skills, and the ability to work collaboratively. Preferred experience in geriatrics, internal medicine, ICU, or emergency medicine, with a valid Nurse Practitioner license and DEA certification.
